Physical Therapist – Home Health (Brevard, Osceola, or Orange County, FL)

Our client is seeking a motivated Physical Therapist (PT) to join a well-established home health therapy group that has been serving Florida patients for over 20 years. This is a full-time role (30 visits/week) with a steady caseload, flexible scheduling, and support from dedicated schedulers.

As a Physical Therapist, you’ll provide high-quality care to a geriatric population , completing SOCs, Evals, RAs, and DCs, while PTAs support routine visits. You’ll have the chance to work with multiple home health agencies in the area, giving you a diverse and rewarding clinical experience.

Key Responsibilities:

• Perform Start of Care, Evaluations, Reassessments, and Discharges
• Provide treatment for home health patients (geriatric focus)
• Document patient visits in agency EMR systems
• Coordinate with schedulers and patients to finalize appointment times
• Ensure high-quality patient care and regulatory compliance

Qualifications:

• Active Florida Physical Therapist (PT) license required
• 1+ year of home health or geriatric PT experience preferred
• Strong organizational and communication skills
• Comfort working with multiple EMR systems
• Availability to work one Saturday per week (flexible schedule: e.g., Tues–Sat)
